General FAQ’s

  • I’m Fernando C. Galvez IV, a solo, independent, non-medical caregiver serving Central Oʻahu to East Oahu. I provide respectful in-home support—companionship, personal care assistance, mobility help, meal prep, medication reminders, light housekeeping, and home-safety checks.

  • Not yet. I currently operate solo and am not a DOH-licensed home care agency. Licensure is in progress; I’m building policies and systems now to meet DOH standards.

  • Yes. I’m liability insured (COI available on request) and I complete voluntary background checks: FBI Identity History Summary and Hawaiʻi eCrim (state convictions).

  • Owner-led care—provided by the owner. As we grow, any future personal care aides will be personally trained by the owner to uphold our mission, continuing client’s care plan, and service standards within a non-medical scope.

  • Oʻahu: Waipahu → Aiea, Hālawa → Kalihi, Downtown, Mānoa → Kahala, ʻĀina Haina → Hawaiʻi Kai. Not sure if you’re in range? Ask!

Care Planning & Process

  • 1) Free phone consult → 2) In-home assessment (goals, routines, safety) → 3) Start care with a written plan and schedule.

  • ADLs/IADLs review, home-safety walk-through, routines, preferences, emergency contacts, and a straightforward plan with visit length and frequency.

  • I review plans at least every 90 days or sooner if needs change.

Scheduling & Visits

  • Yes—so I can arrive on time and prepared. If a day is full, the system hides extra slots; I’ll offer nearby availability.

  • When possible. Same-day and holiday visits may have an additional fee due to scheduling and travel.

  • Rates depend on visit length, complexity, and distance. I provide a transparent quote after the assessment. Hawaiʻi GET applies.

  • Please cancel ≥24 hours in advance to avoid a fee. Short-notice or no-show fees may apply (policy provided in your agreement).

  • Private pay via credit/debit card, ACH bank transfer, or invoice. We also offer a secure client portal for online payments, text-to-pay links, and optional auto-pay (card or ACH). You’ll receive receipts by email, and we can provide itemized statements for long-term care insurance reimbursement.

Scheduling & Visits

  • Yes—concise visit notes and any safety updates. Plans and schedules are kept current in our secure care platform with client portal access on request.

  • We use HIPAA-aware practices and limit what’s collected online (no PHI in public forms). Records are stored in a secure, healthcare-focused system and shared on a need-to-know basis.

Growth & Licensure

  • I’m building the full Department of Health Home Care Agency framework—policies, training logs, Fieldprint-based background checks (state + FBI + APS/CAN), QA processes, and incident reporting—so I can apply and scale responsibly.

  • The core values won’t change. You’ll see expanded availability, DOH-standard procedures, and the same transparency—plus a small, owner-trained team.
